Empower Path Brokerage is an independent OPWDD support brokerage owned and operated by Ebru Altiner, serving families in Suffolk County and Nassau County, New York. This policy is maintained by Empower Path Brokerage and describes how information submitted through empowerpath.life is handled.

Through the contact form on this site we collect only what you choose to enter: your name, email address, an optional phone number, and the message you write. We do not ask for Social Security numbers, Medicaid numbers, diagnoses, or other protected health information, and we ask that you not include them in the form.

Once we begin working together, additional information needed for OPWDD self-direction planning may be collected directly from you or, with your authorization, from your Care Manager or Fiscal Intermediary. That information is handled under the confidentiality rules that apply to support brokerage services.

Contact form submissions are used solely to respond to your inquiry and, if you become a client, to provide support brokerage services. We do not use your information for advertising, and we do not add you to marketing lists.

Form submissions are stored in the secure database that powers this website and are also emailed to ebrualtiner@empowerpath.life so we can reply. Access is limited to Ebru Altiner.

We never sell your information. We share it with third parties only when you ask or authorize us to (for example, coordinating with your Care Manager, Fiscal Intermediary, or OPWDD on your behalf), or where disclosure is required by law.

Inquiries from people who do not become clients are kept only as long as needed to follow up, and are deleted on request. Client records are retained as long as required for the services provided and applicable New York recordkeeping requirements.

This website does not use advertising cookies or third-party tracking profiles. Basic technical data (such as server logs from our hosting provider) may be recorded to keep the site secure and running.

This site is intended for parents, guardians, self-advocates and professionals. We do not knowingly collect information directly from children through this website.
