Who qualifies in Suffolk County
You need an OPWDD eligibility determination and a Care Manager through a Care Coordination Organization. Your Care Manager adds broker start-up services to your Life Plan and names the broker you choose — you are free to choose an independent broker rather than one attached to an agency.
Brokerage is paid through your self-direction budget, so most Suffolk families pay nothing out of pocket.
